From the Nurse
Please note the following and feel free to consult with the School Nurse concerning these issues or any other matters relating to your child’s health and safety, or well-being.
- STUDENT EMERGENCY CARDS MUST BE COMPLETED AND RETURNED PRIOR TO THE START OF SCHOOL. All students are required to have an updated card each school year. It is imperative that the emergency contact card be updated yearly. Please list local people who you authorize to take your child from school when you are not available.
- STUDENT PHYSICAL EXAMINATIONS are required for all 4th grade students. Your private physician’s report of the physical examination is to be sent to the school after completion of the physical examination and prior to the start of school
- STUDENT HEALTH QUESTIONNAIRES MUST BE COMPLETED AND RETURNED EACH SEPTEMBER. All students each year are required to have a yearly updated form. Please answer all questions accurately and completely. Signed form must be returned to the School.

- SCHOOL MANDATED SCREENINGS FOR GRADES FOUR THROUGH EIGHT: Pupils scoring below state guidelines will have written notice sent home.
- Scoliosis Screening to every child ages 10-14
- Hearing tests to grades: 4 - 8
- Vision Screening to grades: 4 - 8
- SCOLIOSIS SCREENING will be completed on all students in grades four through eight. Screening is usually during the months of January and February by the School Nurse. If you wish to have the scoliosis screening done by your private physician, at your own expense, please notify the School Nurse in writing and have a physician’s report of the results in the School Nurse’s office before January 1. A letter pertinent to this screening will be sent home prior to scheduled dates.
- PEDICULUS HUMAN CAPITIS (Head Lice) - This is a Latin name for a common type of head lice. Pediculus is communicable via direct contact, use of personal items, i.e. combs, brushes, and towels, and clothing, i.e. hats, ribbons, and other head apparel. It can happen to anyone, anywhere. It is not a sign of being dirty. Every year the School Nurse will check all students for Head Lice. If a student has Head Lice the School Nurse will notify you and discuss treatment.
- IMMUNIZATION REQUIREMENT: It is mandated by state law that every student completes required immunizations. Please complete and submit immunization record (A45) to the school. Any student not complying with the law will be excluded from school until the requirement has been met. The School Nurse will notify parents in writing of any deficiencies. Parents must bring written records from their physician when immunizations have been administered.
STUDENT ABSENCES: Medical documentation is necessary if absence is due to accident, communicable disease or major illness. Students under the care of a physician for contagious illness i.e., conjunctivitis, strep throat, must remain at home on medication for 48 hours before returning to school. Students sent home with fever should return to school only after their temperature returns to normal after 24 hours and receives medical clearance.
